First, open the .rar file, and drag all thats inside in the resource folder of you The binding of isaac rebirth folder.
This folder is found here:

Windows: C:/Program Files (x86)/Steam/SteamApps/common/The Binding of Isaac Rebirth/resources/

Mac:~/Library/Application Support/Steam/SteamApps/common/the binding of isaac/The Binding Of Isaac.app/Content/Resources/resources/
