I simply replaced Ghost Baby with the co-op Skeleton Baby. This is my first experience with modding, so please tell me if something is misplaced.

But it's worth noting the items.xml is outdated, so if you have Afterbirth or Afterbirth+ the game will crash every time you pick up an item from one of the expasnsions

I don't think so, the mod's not all that popular, and the items.xml would conflict with a lot of other mods that also change it. The mod will still work just fine, it will just still say Ghost Baby instead of Skeleton Baby when you pick it up.
