A lot of people have gotten confused by Mask of Infamy's design after the heart gets destroyed. It's supposed to be a giant Isaac head, similar to The Gate's death animation. Most people though (myself included), originally thought that it was an eyeball. The Concept art, seen here, much more clearly shows this Isaac. I really liked the fetal design and wanted to create a mod that uses that same design.
