This starts the Lost with the D4 and the Push Pin
Unpack the rar, then go to
C:\Program Files (x86)\Steam\SteamApps\common\The Binding of Isaac Rebirth\resources
and replace the players file there with this one.

Why not giving him the item Cupid's arrow, so that the piercing tears effect is permanent?

He's meant to have spectral tears, and not piercing tears. Also the D4 would re-roll any non-trinket items. Push Pin was the best I could do.
