This is a mod which edits a few items and, in my opinion, fixing them to make sense.

This is what I've changed so far:
-Edited all Zodiac signs
-Rainbow baby is now an actual rainbow baby
-Dead cat gives you a dead cat familiar
-A few minimap changes
-Dead bird is upside-down
-The ladder now has been changed into a magical rock ledge that you can use
-Fly items actually look like what they look like
-The mirror now shows how Isaac sees himself
-Transcendence is now a longer rope
-The poop is smelly
-Mom's bottle of pills no longer has two stale pills next to it
-Blood of the martyr actually has blood
-The inner eye has a x3 sign next to it
-Polyphemus is kinda messed up now :p
-Cricket's body matches the color scheme of Cricket's head
-Death's Touch actually matches the color of Death's scythes
-BFFs now is more bloody
-Juicy sack looks more like what you get
-What's a Unicorn Stump? Isn't that an invincible star?
-Maggy's bow is the same color as Maggy's bow
-Some pickup retextures
-A coin slot for the slot machine
-Blood bank actually has blood when you break it
-Burnt penny doesn't really make sense. I tried to make a bomb penny but...it isn't the best...
-Swinger's Maw actually is a Maw
-Change to one load image
-Mama Gurdy's death image no longer has an ugly gray background
-Curse of the Lost's minimap question mark now says "The Lost"
-Pause screen's blood now have more detail
-Both Wraths are just bombs in suits.
-Azazel cries blood
-Lazarus cries blood
-The Lost is now more ghost like
-The Jar now has better graphics
-Cain lost his eye in a cruel way and is now left with a scar
-One arcade background has more blood

UPDATE!
I have added a few new things, nothing major, though:
-Super Wrath is also a bomb in a suit
-Mom's Knife has been used recently...
-Mom's Coin Purse no longer looks like a fly
